Researchers have introduced a new concept in the field of aging and longevity called “peakspan.” This term describes the period during which a person maintains at least 90% of their peak performance in a specific area of health, such as physical strength or cognitive function.

According to a study published in the journal Aging and Disease, peakspan is shorter than most people expect. The research suggests that focusing on extending peakspan, rather than just healthspan, may help people maintain functional abilities for more of their lives.

What is peakspan?

The study defines peakspan as “the age interval during which an individual maintains at least 90% of their peak functional performance in a specific physiological or cognitive domain.” This differs from healthspan, which measures the years a person lives without chronic disease.

A person can have normal lab results and no diagnosable conditions, yet still function well below their cognitive and physical peak. The timelines for healthspan and peakspan do not overlap as much as many assume.

When do people peak?

The research shows that most body systems reach maximum capacity earlier than commonly thought:

Cognition (processing speed, working memory, reasoning) peaks in the mid-20s. Aerobic capacity (VO2 max) peaks between ages 25 and 30. Pulmonary function peaks in the early 20s. Muscle mass and strength peak in the late 20s to early 30s. Immune function peaks during adolescence.

By age 50, a healthy person has likely left peakspan for most physiological and cognitive functions. That person may still have 20 or more years of healthspan remaining. This means someone can be healthy by clinical measures while operating below their former capacity.

The functional gap in midlife

As people age, some may notice that things feel off. They are not sick, their bloodwork looks fine, but they do not feel as sharp, strong, or energetic as before. Researchers call this the functional gap. According to the peakspan framework, this is a measurable reality, not a mystery.

The gap between current function and peak capacity widens each year if left unaddressed. Waiting until disease appears means decades of functional capacity have already been lost. By the time a person is considered unhealthy by clinical standards, the decline has been happening for years.

How to extend peakspan

The study identifies several evidence-based ways to extend peakspan. Resistance training preserves muscle mass, strength, and metabolic function. Aerobic exercise supports VO2 max, cardiovascular health, and brain function. Even modest improvements in aerobic capacity can extend functional years.

Cognitive challenges such as novel learning, problem-solving, and mental stimulation help maintain fluid cognition longer. Sleep optimization supports immune function, cognitive performance, and muscle recovery. Nutrition that includes anti-inflammatory eating patterns, adequate protein, and targeted nutrients supports cellular health and functional capacity.

Researchers note that interventions are most effective when started early, but early is relative. Starting at 40 is better than starting at 50. Starting at 50 is better than starting at 60. The key is recognizing that optimization, not just disease prevention, should be the goal.

The peakspan concept reframes how people think about aging. It is not just about adding years to life or avoiding disease. It is about maintaining functional capacity throughout the many systems of the body, which makes those years worth living.
